Area Agency on Aging Funding
The Area Agency on Aging for Southwest
Florida has contracted for an array of community based long term
care services within the geographic area of Charlotte, Collier,
DeSoto, Glades, Hendry, Lee, and Sarasota counties. Funding for
these services originates from the following sources and is
allocated through the Florida Department of Elder Affairs:
Older Americans Act (Federal): Provides funding for access, in home nutrition, health and wellness programs. Services include: information and referral, outreach, case management, transportation, chore, homemaker, legal assistance, congregate and home delivered meals, and health screening. Eligibility: 60 years of age or older. While no fee is charged for services, contributions are encouraged.
Community Care for the Elderly (State): Provides similar services to that of the Older Americans Act, however individuals served have higher physical impairment levels. Services include: case management, adult day care, respite, emergency alert response, personal care and medical transportation. Eligibility: 60 years of age and older and physically impaired. A sliding scale is used in determination of fee.
Alzheimer?s Disease Initiative (State): Provides respite services for individuals caring for Alzheimer?s patients. Programs in selected counties offer respite in the home, in a group setting, or temporarily in a facility. Eligibility: Diagnosed with Alzheimer?s Disease or other dementia. A sliding scale is used in determination of fee.
Medicaid Waiver/Home & Community Based Care (Federal/State): Provides elderly and disabled adults determined eligible for Medicaid nursing home care with an option of receiving intensive care management and in-home supportive services to prevent or delay institutionalization. Eligibility: Must meet both income and other eligibility factors established in Medicaid guidelines.
